All items shipped to the Spa must be accompanied by a completed Spa Services form. This form describes the item or items being shipped and the services requested. Upon receipt of your shipment by the Spa, our staff will verify the condition of the item received and return the counter-signed form to you via email. Please remember to sign the form at the bottom.
